LPN

Brookside Extended Care is a 92 bed ICF/IID facility that is licensed through the Ohio Department of Developmental Disabilities and receives oversight from the Ohio Department of Health. Brookside currently has full-time and part-time positions available on “B” shift (5:30pm-5:30am).  Apply today and make a difference in the lives of the individuals we serve.

Duties include, but are not limited to:

• The administration and documentation of prescribed medications and treatment
• Assessing and monitoring health status
• Providing timely and appropriate nursing care
• Transcribing physician’s orders according to Nursing Procedures
• Maintaining detailed nursing notes
• Participating in the monthly changeover of medications and treatments
• Working collaboratively with the interdisciplinary team to ensure quality care is provided
• Providing direct care to individuals as needed
• Assisting with the supervision of direct care staff

Job Requirements:
•Candidate must be at least 18 years of age
•Must be licensed by The State of Ohio as a Licensed Practical Nurse
•Experience working with individuals with disabilities preferred
•Positive attitude and the ability to work with others as a team
•Basic computer skills
