PRN or Full-time
AM Shift 6:30 AM - 7 PM or PM Shift 6:30 PM- 7 AM
Community First Solutions has three, 5 Star facilities Berkeley Square, Westover & Jamestowne
Promote resident Wellness by fostering a person-focused approach to care; guide members of the nursing team to provide care and services using current best practices while operating within local and federal regulations; collaborate with physicians and other interdisciplinary team members to address resident concerns and develop a resident-driven Wellness Plan.

 

Works from a team approach to meet residents needs.  Assesses and reports physical, mental and emotional changes in residents' condition.  Plans, prioritizes and initiates resident care.  Administers medications and treatments as ordered by the physicians.  Maintains supervision of care provided by the Resident Assistants.  Documents pertinent information using established policies/procedures and guidelines.

 

 

 

Job Requirements

Minimum Job Requirements:

Education:  Graduate from an Ohio Board of Nursing approved nursing program or school of nursing. LPN’s must show evidence of successful completion of Pharmacology course.

Licenses or other required certifications: Must have current license to practice nursing in the State of Ohio, hold current CPR card.

Experience:  Previous experience in long term care/geriatrics preferred.

Specialized knowledge, skills, or abilities: Familiar with infection control practices, OSHA guidelines relative to long term care, and Ohio Department of Health Regulations.
