Caregiver

To provide direct services to persons accessing the supported living program or Medicaid waiver services in accordance with supported living philosophy and the Individual Service Plan.

Complete all necessary daily documentation per guidelines of the Medicaid waiver program and company policies

Monitor health and safety issues or persons in the household

Assist the persons in the home to administer and document the administration of medications and treatments according to physician orders

Participate in scheduled training and certification renewal as required

Maintain driving eligibility per company policies and provide transportation services as needed

Maintain a professional and positive manner at all times

Participate in Quality Assurance Reviews for the Supported Living program

Assist persons in the home to maintain a clean and safe household

Other duties assigned by supervisor

 

High school diploma or equivalent required

Experience working with persons with developmental and/or physical disabilities is preferred

Valid driver's license, good driving record, proof of full liability insurance, and reliable transportation

Be free from any disqualifying offenses which would prohibit individual from providing care for a person with a developmental disability
