Description
Background: Join the Butler County Ohio Equity Institute (OEI) established in 2013 as the
Butler County Partnership to Reduce Infant Mortality (PRIM). PRIM works to improve overall
birth outcomes and reduce the racial and ethnic disparities in infant mortality in Butler County.
Essential Functions and Goal: Under the general direction of the Ohio Equity Institute/PRIM
Project Coordinator, the Neighborhood Navigator works to help reach Butler County Health
District’s mission, and will identify and engage unserved at-risk pregnant women and connect
them to comprehensive clinical care and social services. The Navigator will facilitate access to
services by acting as a liaison (bridge) between health and social services and the Butler County
community. The Neighborhood Navigator will develop and maintain a resource portfolio of
services to facilitate linking at-risk women to services. The goal of this position is to improve
birth outcomes for women served by the Navigator. Program goal is that 80% of women served
are African or African-American women, in specific zip codes where inequities in birth
outcomes are most pronounced.
Job Requirements
Minimum Qualifications:
High School Diploma or its equivalent.
Basic computer skills.
Passionate about improving maternal and infant health.
Strong interpersonal and communication skills and the ability to work effectively with a
diverse community are essential.
Interpersonal skills required include professionalism, sensitivity, empathy, flexibility,
accessibility, patience, resourcefulness, enthusiasm, and the ability to be non-judgmental.
Exhibit excellent working relationships with women, visitors and staff, effectively
communicating Butler County Health District’s mission.
This position requires the candidate to have a reliable personal vehicle, and a valid Ohio
driver’s license in good standing and insurable.
Qualifications Preferred:
Successful completion of a Certified Community Health Worker formal training program, or
health/social service related degree
Butler County resident or knowledgeable of Butler County resources.
Knowledge of infant mortality reduction concepts, social determinants of health and health
disparities.
Experience working in a community based setting for at least 1-2 years.
Bilingual or multilingual (French or Spanish) helpful
Primary Duties Include (but not limited to):
Responsible for establishing trusting relationships with women and their families while
providing general support and encouragement.
Conduct intake interviews with women including use of the ODH-identified screening tool.
Maintain/develop personal knowledge base about community resources appropriate to needs
of women/families.
Develop and maintain a portfolio of local referral resources to ODH-funded services,
evidence-based interventions, clinical and social services.
Connect and refer women to applicable programs and services.
Provide follow-up to women via phone calls and other means as necessary to ensure their
needs are met.
Document woman/family referral and address risk status via the ODH-identified platform.
Attend regular staff meetings, trainings and other meetings as requested.
Collaborate with Ohio Equity Institute/PRIM Project Coordinator to review documented
barriers and access challenges experienced by women served by Navigator.
Other:
Serves a probationary period of 120 days
