We have 2nd and 3rd shift differentials
Position Summary
The LPN is responsible for facilitating nursing floor operations in accordance with federal state and local regulations and standards, ensuring that each resident receives quality care. Encourages each resident’s ability to maintain or attain the highest practical, physical, mental, and psycho-social well-being. This position requires excellent communication skills with team members, residents and family members as well as a high level of patience and compassion for others.

Essential Functions

  • Gives and receives the nursing report upon the start and end of each shift.
  • Directs day-to–day job function duties of resident assistants (STNA, PCA). Assists with feeding as needed.
  • Provides individualized attention, implements plan of care, dispenses medication, and takes vital signs as ordered. Reviews and documents activities via MARS, TARS and AOD .
  • Performs all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials.
  • Admissions, transfers and discharge functions as assigned.
  • Communicates with Doctor(s).

Job Requirements

Required Education and Experience
Must possess current LPN License from the state of Ohio
Diploma from accredited nursing school
Previous nursing experience in a continuing care retirement community or hospital preferred
Must possess a customer service background and orientation with demonstrated problem solving skills
Computer proficiency
