POSITION SUMMARY:
The Vocational Services Case Manager will work as a part of a larger team of clinicians and social workers coordinating care and service for adolescents and adults. The Vocational Services Case Manager will help identify barrier to employment and assist in their removal, empower and develop skills in order to more clients forward in education or career pathway. The Social Services Case Manager will work with the TANF, Temporary Assistance for Needy Families program to help needy families achieve self-sufficiency.
ESSENTIAL JOB FUNCTIONS:
- Maintain caseloads and productivity by ensuring regular contact with clients in community and office by collaborating with clients to identify their strengths and areas of interest.
- Work with local agencies like the Department of Vocational Rehabilitation or the Department of Workforce Services to determine what services and programs the client might be eligible to participate.
- Monitor and assess progress per agency policy, regulatory requirements and best practices.
- Meet with clients individually or in a group to assess needs; collaborate with clients, families and other case managers and clinicians.
- May provide, but is not limited to: crisis intervention, advocacy, medication monitoring, transportation assistance, financial assistance, ADL assistance, referral and linkage to community resources. May visit, monitor and transport clients.

Job Requirements
QUALIFICATIONS REQUIRED:
• Bachelor’s degree in Behavioral Science or related area or; equivalent combination of experience and training
• Previous Case Management Experience
• Complete an internal training and pass a competency test on electronic clinical documentation software during first eight weeks of employment
• A valid driver’s license and current auto insurance may be required

QUALIFICATIONS PREFERRED:
• Knowledge of TANF, JFS work participation rules
JOB SPECIFIC REQUIREMENTS:
• Must remain organized and must prioritize caseloads and communicate effectively to clients, clinicians, case managers and families.
• Must be able to work independently
• Experience with networking and developing resources for clients
• Must be able to climb stairs (6 flights)
• Must have driver's license, auto insurance and own vehicle
